Home Care Process

We come to you regardless of where you are for the assessment of care – in your home, in the hospital or in a residential care facility.  This is when you and our coordinator will work through what services you require from us in your home.

Following the assessment, a Clinical Co-ordinator will; in conjunction with our clients and their families, develop a care and wellness plan individually tailored and responsive to specifically identified needs, problems and goals.

We will select the carer most suited to meet each client’s needs and preferences and, if possible, arrange a personal introduction.

The services you require will be delivered on the days and at the times most convenient to you.

All clients benefit from our 24 hour support service, which allows for peace of mind should unexpected needs arise.
